Welcome to our GAMEDAY Survey Site, an online survey site offered by Frank N. Magid Associates, Inc.  The survey site (the “Service”)  is operated by Frank N. Magid Associates, Inc., 8500 Normandale Lake Blvd, Suite 630, Minneapolis, MN 55437 (“Magid” or “we”).

For your child to use the Service, you are required to consent to the collection, use and disclosure of information from your child as described below.  Magid and its vendors will not collect, use or disclose any personal information from your child if you do not provide such consent.  Without your consent, your child will not be able to use the Service.

If your child begins using the Service, your child’s zip code, country, age, and gender will be collected in addition to the information they provide while using the Service (such as answers to survey questions on topics such as favorite sports teams and use of the NFL RUSH Gameday app).  The information collected will be used to obtain your consent to your child’s use of the website to take the surveys, to generate and provide survey results to the provider of the NFL RUSH Gameday app, to update you on changes to our Children’s Privacy Policy, and to assist you with customer service inquiries and technical support.

Any vendors that assist Magid with the Service may also have access to the above information and have agreed to comply with the Magid privacy policy.
